Your lungs and gut share more than you'd think. A two-way connection may explain breathing problems many people never link back to digestion.

How your gut and lungs stay in contact
There's a genuinely direct research trail connecting gut bacteria to lung disease, built on one of the largest studies in this field. A Finnish study followed 7,115 adults for up to 15 years. The mix of bacteria in a person's gut at the start of the study helped predict who would go on to develop asthma or chronic obstructive pulmonary disease (COPD), sometimes years before any symptoms appeared. Researchers call this two-way relationship between gut and lungs the gut-lung axis.

Why the gut-lung connection matters
The gut and the lungs start out as the same tissue before birth, and they stay in contact throughout life. Gut bacteria ferment fibre into short-chain fatty acids, compounds that travel through the bloodstream and help calm inflammation in the lungs. When gut bacteria are disrupted, a state researchers call dysbiosis, this calming supply can drop, and airway inflammation can become harder to control.
This connection runs in both directions. A 2024 study exposed mice to cigarette smoke for 20 weeks and found disrupted bacteria in the gut and the lungs at the same time, with the two sets of changes linked to each other. A separate 2025 meta-analysis pooling 21 studies found a consistent pattern in COPD specifically, covering 1,273 people with COPD and 6,718 healthy people. People with COPD had less diverse, disrupted gut bacteria than people without the disease.
This shows up in practical ways too. A large US survey found that people who regularly consumed probiotics, prebiotics, or yoghurt had a 25% lower prevalence of COPD than people who did not. In children, a Canadian study found that specific gut bacteria patterns at just three months old were linked to a higher risk of developing asthma by age one, suggesting the gut's influence on the lungs can start very early in life.
This pattern is not limited to COPD or early asthma. It appears in the gut bacteria of adults who smoke, in the shifting microbiome seen as COPD becomes more severe, and in the breathing problems that can linger for months after a respiratory infection such as COVID-19. What this means for you
None of this replaces medical treatment for a lung condition. Inhalers, medication, and stopping smoking where relevant remain the main tools for managing lung disease. Persistent breathlessness, a cough that will not go away, or coughing up blood need medical assessment.
