The Real Gut Cost of Treating a Viral Cough


There's little direct research on bronchitis and the gut, but the indirect route here is well established and worth taking seriously.


Acute bronchitis is inflammation of the bronchi, the airways that carry air into the lungs, and it's usually caused by a virus rather than bacteria.


The gut connection isn't about the infection itself. It's about what commonly gets prescribed to treat it.


Most bronchitis prescriptions treat a virus antibiotics can't touch


Antibiotics only work against bacteria, not viruses, yet a large study of primary care visits found antibiotics were prescribed unnecessarily in about 74% of acute bronchitis cases, the highest rate of any common respiratory diagnosis studied.


Other research puts the figure at 60 to 80% across different healthcare settings and time periods.


Acute bronchitis has consistently been the single condition most likely to receive an antibiotic prescription it doesn't need.

A single course of antibiotics can reshape gut bacteria for months


Broad-spectrum antibiotics, medications designed to kill a wide range of bacteria rather than one specific type, don't distinguish between harmful bacteria and the beneficial species living in the gut.


A classic study tracking three people through two courses of the antibiotic ciprofloxacin found gut bacterial diversity dropped sharply within three to four days of starting the drug.


Communities began to recover within a week of finishing each course, but the recovery was often incomplete. After the second course, each person's gut bacteria settled into a new, altered state and stayed there for at least two months, the entire length of the follow-up.


This disruption is a known risk factor for other health problems, since the gut's bacterial community usually helps keep the whole ecosystem in balance.

Losing that protection opens the door to opportunistic infections


A healthy gut community provides what's known as colonisation resistance, where the resident bacteria physically crowd out harmful species and make it harder for them to take hold.


When broad-spectrum antibiotics wipe out large parts of this community, that protection weakens. One well-documented consequence is Clostridioides difficile infection, a gut infection that causes severe diarrhoea and is directly linked to the loss of colonisation resistance after antibiotic treatment.


This is the real cost sitting behind an unnecessary bronchitis prescription. It isn't just a wasted tablet, it's a genuine disruption to a working ecosystem.

If antibiotics genuinely are needed, there's a way to offset some of the damage


When antibiotics are truly necessary, for a confirmed bacterial infection rather than a viral cough, taking a probiotic alongside them has a measurable protective effect.


A meta-analysis of 42 randomised trials in adults found probiotics reduced the risk of antibiotic-associated diarrhoea by 37%, and multi-strain probiotics worked better than single-strain ones.


Adding bacteria alone isn't the full story though. A probiotic strain still needs feeding with fibre and plant compounds afterwards to establish itself in the gut, otherwise it won't survive there long enough to help.


The fact that multi-strain probiotics outperformed single strains fits this pattern. A more diverse bacterial community appears to hold up better than one narrow addition.
