The Gut Bacteria Losses Behind a Lung Infection
The evidence connecting the gut to pneumonia is moderate, built on a run of consistent studies rather than one landmark trial.
Pneumonia is a lung infection where the air sacs fill with fluid or pus, making it hard to breathe, and it kills more people worldwide than any other infectious disease.
People hospitalised with pneumonia show a measurably less diverse set of gut bacteria than healthy people, and that difference tracks with how inflamed their blood becomes.
Gut bacteria change while the lungs are still fighting the infection
A study comparing people hospitalised with community-acquired pneumonia (a lung infection picked up outside hospital) to healthy people found significantly lower gut bacterial diversity in the pneumonia group.
Genera that produce short-chain fatty acids (SCFAs), compounds gut bacteria make by fermenting fibre that calm inflammation and support the gut lining, dropped noticeably. Two examples, Blautia and Agathobacter, were both reduced.
At the same time, blood levels of lipopolysaccharide (LPS), a bacterial fragment that leaks from a weakened gut lining, along with two inflammatory markers called TNF-alpha and IL-6, were all significantly raised.

This pattern shows up across respiratory infections generally
A systematic review pooling multiple studies found gut bacterial diversity was, on average, 1.45 units lower in people with a respiratory tract infection than in healthy people.
The review found a consistent loss of Lachnospiraceae, Ruminococcaceae and Ruminococcus, three groups of fibre-fermenting bacteria, alongside a rise in Enterococcus, a genus more often linked to inflammation.
This wider pattern suggests pneumonia isn't an isolated case. It fits a broader link between a weakened gut community and how the body handles infections in the lungs.

Mouse studies suggest the gut bacteria actively help fight it off
A study in mice tested this directly rather than just observing it. Researchers depleted the gut bacteria of healthy mice, then infected them with Streptococcus pneumoniae, the bacterium behind most bacterial pneumonia.
Mice without healthy gut bacteria showed weaker lung immune cells, called alveolar macrophages, that were less able to engulf and destroy the invading bacteria. Genetic analysis of these cells showed altered metabolic activity and a reduced response to bacterial fragments they would normally recognise and attack.
When researchers restored gut bacteria in these mice through a faecal microbiota transplant (FMT), a procedure that transfers stool from a healthy donor to reintroduce gut bacteria, lung bacterial counts and two inflammatory markers returned close to normal within six hours.
This is stronger evidence than an observational study alone, because it shows removing and restoring gut bacteria directly changed how well the lungs coped with infection.

What this means for you
Pneumonia needs medical assessment, especially with breathing difficulty, chest pain, or a high fever, since some cases need antibiotics or hospital care.
