Why breathing can take months to recover
Around half of people hospitalised with COVID-19 do not fully recover within three months, and their gut bacteria show a specific, measurable pattern behind that slow recovery. A study tracked 146 people with COVID-19 against 110 healthy people, using genetic sequencing of stool samples, following patients for up to three months. Gut bacteria diversity dropped sharply during hospitalisation, then partially recovered over the following months, roughly tracking how quickly people's own symptoms improved.

What the gut bacteria show
A separate, larger study followed 106 people with COVID-19 for six months, comparing them with 68 people who had not had the infection. At six months, 76% of the COVID-19 group still had at least one lingering symptom, most often fatigue, poor memory, or hair loss. People whose symptoms had resolved by then had gut bacteria that looked similar to people who had never had COVID-19. People with lingering symptoms did not. Their gut bacteria showed higher levels of Ruminococcus gnavus and Bacteroides vulgatus, and lower levels of the butyrate producer Faecalibacterium prausnitzii.
Specific symptoms tracked with specific bacteria. People with ongoing breathing problems had higher levels of several Streptococcus species and a bacterium called Clostridium disporicum in their gut. People with brain fog and fatigue instead had higher levels of different bacteria, more commonly linked to hospital-acquired infections. Gut bacteria measured right when patients were first admitted to hospital could help predict who would still have symptoms six months later, before those symptoms had even appeared.
This pattern fits with what researchers see in other lung conditions too. Faecalibacterium prausnitzii and similar depleted bacteria are known producers of short-chain fatty acids, compounds that travel through the bloodstream and help calm inflammation in the lungs. A gut still short of these bacteria months after infection may mean a lower ongoing supply of that calming effect, at the same time the lungs are trying to repair themselves.

What this means for you
Lingering breathing problems after an infection should be reviewed by a doctor, especially if breathing has not clearly improved after several weeks, since some causes need specific treatment. Supporting the gut through this recovery period is a reasonable, low-risk habit to pair with medical follow-up, not a substitute for it.
