Why Sugar and Its Substitutes Aren't the Same Story for Gut Bacteria
There's a genuinely strong, causally tested finding here for artificial sweeteners specifically, alongside solid, if more general, evidence for sugar itself.
Both added sugar and artificial sweeteners can reshape gut bacteria, but through different routes and with some genuinely surprising results.
The sweeteners marketed as the "safe" alternative to sugar turned out to have their own distinct, and unexpected, effect on the gut.
A landmark study found sweeteners, not sugar, caused glucose intolerance in mice
Researchers gave healthy mice either plain water, water with real sugar, or water with one of three artificial sweeteners, saccharin, sucralose, or aspartame, for several weeks.
The mice drinking artificial sweeteners developed glucose intolerance, a state where the body handles blood sugar less effectively, while the mice drinking actual sugar did not. Saccharin produced the strongest effect of the three sweeteners tested.
To confirm gut bacteria were actually responsible, researchers gave the sweetener-fed mice antibiotics to clear their gut bacteria. This reversed the glucose intolerance, and transplanting stool from sweetener-fed mice into germ-free mice, animals with no gut bacteria of their own, reproduced the same glucose intolerance in the new mice.

The response to sweeteners varies enormously between people
Follow-up human research found something equally important. Not everyone responds to sweeteners the same way. In one trial, a subset of participants, labelled "responders", showed clear microbiota changes and worsened blood sugar handling after saccharin, while others showed no response at all.
The gut bacteria of the responders looked distinctly different even before they started taking the sweetener, suggesting some people's existing gut community makes them more susceptible to this effect than others.
This kind of individual variation is a genuine, replicated finding, not just noise in the data. It means broad statements about sweeteners affecting "the gut microbiome" in general somewhat overstate how uniform the effect actually is.

Sugar itself has its own, separate effect on the gut
Setting sweeteners aside, high intake of sugar itself, including glucose, fructose, and sucrose, has been consistently linked to reduced gut bacterial diversity and a shift toward less favourable bacterial groups in both animal and human studies.
Research specifically found high sugar intake increases Proteobacteria, a bacterial group linked to inflammation, while decreasing Bacteroidetes, a group that helps maintain a strong gut barrier and limit the effects of bacterial toxins.
This can weaken the gut lining and contribute to low-grade inflammation over time, entirely separate from the artificial-sweetener story above. Sugar and sweeteners disrupt the gut through genuinely different routes, not the same mechanism wearing two different labels.

What this means for you
There's no need to treat every sweetened drink or dessert as a crisis, these effects build up with sustained, heavy intake rather than occasional treats. What the research does make clear is that neither sugar nor its artificial substitutes are a neutral choice for gut bacteria, and swapping one for the other isn't automatically the gut-friendly move it's often assumed to be.
