Why the Liver Is the Organ Most Exposed to the Gut


The liver receives about three quarters of its blood supply directly from the intestines, through a vessel called the portal vein. Everything absorbed from the gut, and everything leaking across its wall, arrives at the liver first and at full concentration before reaching anywhere else in the body. No other organ sits in that position. That single fact of anatomy is why gut bacteria turn up in the research on almost every liver and gallbladder condition, and why the evidence here is unusually strong.


What arrives at the liver, and what it does


Some of what reaches the liver is manufactured by gut bacteria from food. In fatty liver disease, up to 60% of patients in one Chinese cohort carried a strain of Klebsiella pneumoniae producing high levels of alcohol from carbohydrate. Transferring that strain to healthy mice gave them fatty liver. Removing just that one strain from a transplanted community prevented it.


Some of it is bacterial debris. When the gut wall loosens, endotoxin from bacterial cell walls crosses into the portal vein. The liver's resident immune cells recognise it and respond, and 80% of endotoxin entering the bloodstream is found in the liver within half an hour. Sustained, that response both inflames the liver and instructs it to lay down scar tissue.


Some of it is chemistry the liver started. Bile acids are made by the liver, sent into the gut, modified by bacteria and largely returned. Those modifications determine whether cholesterol stays dissolved in bile, which is what decides whether gallstones form. Transplanting gut bacteria from people with gallstones into stone-resistant mice made stones form.


As liver disease advances the relationship inverts. In cirrhosis, more than half the bacterial species found in the gut normally live in the mouth, having survived the journey because stomach acid and bile no longer stop them.

What this means for you
