Why Immune Training Happens in the Gut
Roughly seventy per cent of the body's immune cells sit in the wall of the intestine. That isn't an accident of anatomy. The gut is where the immune system meets the outside world in the largest quantity. It is also where immune cells learn the difference between something that needs attacking and something harmless. Get that training wrong in one direction and the result is allergy. Get it wrong in the other and the result is autoimmunity or an immune system that fails to respond at all.
One organ, four different failures
Allergy is the immune system attacking something harmless. The most striking result in this field came from a peanut allergy trial, where 90% of children given a specific probiotic alongside peanut immunotherapy became desensitised, against 7% on placebo. Hay fever has been tested more broadly, and a meta-analysis of 30 trials covering more than 2,700 people found probiotics genuinely improved symptoms and quality of life, though not every measure moved.
Autoimmunity is the immune system attacking the body itself. In rheumatoid arthritis, one gut bacterium is found in 75% of people with newly diagnosed disease against 21% of healthy people. In lupus, a specific bacterium has been observed blooming immediately before flares, tracked in the same patients over repeated samples. Across several autoimmune conditions, tests of gut wall permeability read measurably higher than in healthy people.
Weak immune responses are the third failure, and a six-month trial in children with recurrent respiratory infections found a two-strain probiotic significantly reduced how often and how long infections lasted.
The fourth is chronic inflammation, where the immune system runs hot without a target. A meta-analysis of 10 trials found dietary fibre significantly lowered C-reactive protein, a standard marker of that state.
Honest results belong here too. A twelve-month trial in care home residents found only limited effects on immune markers, which is worth knowing alongside the positive findings.
