Why oral health reaches beyond the mouth


There's strong evidence that the mouth's own microbiome affects health well beyond the mouth itself. The mouth is the first part of the digestive system. It has its own microbiome, a community of bacteria, fungi, and viruses. This is the second largest microbial community in the body, after the gut. When it's disrupted, this is linked to tooth decay, gum disease, bad breath, and oral thrush. But researchers have found the effects don't stop there.


The heart connection


A large meta-analysis of 39 cohort studies, covering more than 4.3 million people, found that gum disease was linked to a 14% higher risk of heart attack and a 26% higher risk of stroke, compared with people who didn't have it. Researchers believe oral bacteria can enter the bloodstream directly. Once there, they can damage the lining of blood vessels. The body's immune response to these bacteria can also trigger widespread inflammation, the same kind of inflammation that drives atherosclerosis, the plaque buildup behind most heart attacks and strokes.


In a striking 2019 finding, researchers examined blood clots removed from 75 patients during emergency stroke treatment. Inside those clots, they detected DNA from a group of oral bacteria called viridans streptococci, and at levels far higher than in the patients' own blood samples. This was the first study to show oral bacteria commonly present inside the actual clot causing a stroke. Before this, oral bacteria had only been linked to stroke risk statistically, not found at the physical scene of the event itself.

Treating the gums may help the heart too


Several clinical trials have looked at what happens when gum disease is treated intensively. A systematic review of trials in people with high blood pressure found that periodontal treatment lowered systolic blood pressure by an average of 12.5 mmHg and diastolic blood pressure by 10.0 mmHg, alongside reductions in inflammatory markers like CRP. This suggests the oral-heart relationship runs in both directions. Treating the gums can lead to measurable improvements in cardiovascular markers too, not just oral ones, though a major American Heart Association statement in late 2025 was careful to note that a direct cause-and-effect relationship, and proof that treating gum disease itself prevents heart attacks, is still not established.
