Genetic studies have linked specific gut bacteria to tinnitus, vertigo, and hearing loss, part of what researchers call the gut-inner ear axis.

A real gut-inner ear connection
Researchers have found genetic evidence linking gut bacteria to several ear and hearing conditions, enough that they now call it the gut-inner ear axis. A study using genetic data from 18,340 people's gut bacteria and hearing records from more than 331,000 people found six specific gut bacteria causally linked to sensorineural hearing loss. A separate genetic study found specific bacteria linked to tinnitus risk, and another found relationships between gut bacteria and two distinct types of vertigo.

The evidence varies by condition
This connection is strongest where genetic and large cohort studies exist. Hearing loss, tinnitus, and vertigo all have genetic studies directly testing whether specific gut bacteria play a causal role. Balance problems have their own strong evidence too: a study of 2,081 older women, replicated in a separate group of 1,448 people, found gut bacteria diversity directly predicted the risk of falls, hip fractures, and death. Ear infections have real clinical trial evidence behind specific probiotic strains, with results that genuinely depend on which strain is used. Earwax blockage is the one clear exception, with little direct gut-specific research at all so far. The common thread across the stronger cases is inflammation: gut bacteria produce compounds that calm or worsen inflammation elsewhere in the body, including in the structures involved in hearing and balance.

A pattern across conditions, not just one
One bacterium keeps reappearing across this research in different forms. A butyrate-producing bacterium called Faecalibacterium prausnitzii showed up as protective in the large falls study, and butyrate itself, the compound this bacterium produces, has been shown separately to calm the same kind of airway and immune inflammation implicated in several of these conditions. Meanwhile, Parabacteroides distasonis showed up as protective for tinnitus in both a large genetic study and a completely separate mouse experiment, a rare case of the same specific bacterium being confirmed in two very different types of research.
This repetition across conditions and study types is part of why researchers take the gut-inner ear axis seriously, rather than treating each condition as an isolated coincidence.
