What Actually Happens to Gut Bacteria During a Course of Antibiotics
There's a genuinely strong, well-documented body of evidence here, built on studies tracking real people through real antibiotic courses.
Antibiotics work by killing or disabling bacteria, and broad-spectrum antibiotics, the kind most commonly prescribed, don't distinguish between harmful bacteria and the beneficial species living in the gut.
The scale and duration of the resulting disruption is often larger than people assume.
Diversity drops within days, and doesn't simply bounce straight back
A landmark study tracked three people through two separate courses of the antibiotic ciprofloxacin, taking regular gut bacteria samples throughout. Diversity dropped sharply within three to four days of starting each course.
Communities began recovering within about a week of finishing the antibiotics, but the recovery was often incomplete. After the second course specifically, each person's gut bacteria settled into a new, altered state, and stayed there for at least two months, the full length of the study's follow-up.
This wasn't a one-off finding either. Different people responded somewhat differently to the same antibiotic, but the pattern of incomplete, individualised recovery held across all three.

Losing that protection creates an opening for other problems
A healthy, established gut community provides what's called colonisation resistance, where resident bacteria physically crowd out incoming species and make it harder for them to take hold.
When broad-spectrum antibiotics clear out large parts of this community, that protection weakens. One well-documented consequence is Clostridioides difficile infection, a gut infection causing severe diarrhoea that's directly linked to the loss of colonisation resistance following antibiotic treatment.
This is the less-discussed cost of antibiotic treatment. It's not just about which bacteria get killed during the course itself, it's about what's left standing, or not standing, to defend the gut once treatment ends.

Some courses of treatment cause deeper, longer-lasting damage than others
Not all antibiotic regimens are equal. Multi-drug combinations taken over months, such as the standard four-antibiotic treatment for tuberculosis, cause a uniquely severe disruption compared with any single antibiotic tested alone.
Research following tuberculosis patients found the altered gut community was still detectably different from healthy people's microbiomes nine months after treatment finished, with some changes persisting up to two years in certain studies.
The general pattern holds across contexts: longer courses, and combinations of multiple antibiotics, tend to cause deeper and slower-recovering disruption than a short, single-antibiotic course for a minor infection.
