A striking bacterial signature at disease onset
Rheumatoid arthritis, an autoimmune condition causing joint inflammation and damage, has one of the more consistent gut bacteria links of any autoimmune disease. Prevotella copri, a specific gut bacterium, is found in around 75% of people with new-onset rheumatoid arthritis, compared with roughly 21% of healthy people, and this enrichment shows up even before diagnosis, in people at increased genetic risk who haven't yet developed the disease.

A plausible mechanism, not just a coincidence
Researchers have identified a specific way this bacterium may contribute to disease. Some gut bacteria, including strains linked to Prevotella copri, can convert the amino acid arginine into citrulline, and citrullinated proteins are a well-known trigger for the immune response that drives rheumatoid arthritis, since testing for antibodies against citrullinated proteins is already a standard diagnostic tool. This gives a genuine, testable mechanism for why this particular bacterium might drive disease, rather than just correlating with it.
Probiotics show a real, if modest, effect on symptoms
A meta-analysis of randomised controlled trials found that probiotic supplementation modestly reduced disease activity scores and inflammatory markers, including a meaningful reduction in C-reactive protein and interleukin-1 beta, in people with rheumatoid arthritis. Individual trials using strains like Lactobacillus casei have found reduced pro-inflammatory cytokines and disease activity after 8 weeks of supplementation. As with any probiotic, these effects depend on the bacteria being properly fed, since a strain introduced without adequate fibre and plant compounds has far less chance of shifting a gut environment that may already favour inflammation-promoting bacteria like Prevotella copri.
