A study of 2,081 older women found specific gut bacteria directly predicted their risk of injurious falls, replicated in a separate cohort of 1,448 people.

Gut bacteria predict who actually falls
There's strong evidence linking gut bacteria to fall risk specifically, not just balance in general. Researchers followed 2,081 women aged 75 to 80 for almost 8 years, tracking their gut bacteria alongside injurious falls, hip fractures, and death. People with the most disrupted gut bacteria had a 63% higher risk of an injurious fall than those with healthier gut bacteria. Their risk of hip fracture was more than double, and their risk of death within the study period was five times higher. In total, 404 specific bacterial species were significantly linked to this risk, and lower gut bacteria diversity on its own was directly tied to a higher risk of both falls and death, independent of age or existing health conditions.
This wasn't a one-off finding. Researchers repeated the analysis in a completely separate group of 1,448 older adults in China, and found the same pattern held up. People with lower gut bacteria diversity, and lower levels of a beneficial, butyrate-producing bacterium called Faecalibacterium prausnitzii, consistently had worse physical function and higher mortality in both countries.

Specific bacteria stood out
A handful of harmful bacteria kept showing up in people with worse outcomes, including species from the Enterocloster group and one called Eggerthella lenta. These bacteria were linked to lower physical function and a higher risk of death in both the original study and the Chinese replication. On the other side, higher levels of Faecalibacterium prausnitzii, a bacterium that produces a compound called butyrate, tracked with better physical function and longer survival. Looking at what the bacteria were actually doing, not just which species were present, told a similar story. People with more frailty had gut bacteria doing more amino acid breakdown and less butyrate production, a functional shift that lined up with the specific bacteria found and with worse physical performance on standard tests like walking speed and chair-stand ability.
Animal studies point to a real mechanism
Separately, researchers studying mice with an induced vestibular deficit, meaning damage to the inner ear system that controls balance, found measurable changes in gut bacteria and the chemical compounds those bacteria produce. Balance problems and anxiety often occur together, and this research found a possible shared cause: when helpful anti-inflammatory bacteria were depleted, the mice showed higher levels of inflammatory chemicals in their blood and brain, a pattern already linked to anxiety in other research. In a separate experiment, giving butyrate directly to mice on a high-fat diet reduced their anxiety-like behaviour, adding weight to the idea that this specific gut compound has a real, calming effect on the same inflammatory pathway.
