Why Tendon Problems Are Partly Metabolic


No study has examined gut bacteria in tendonitis specifically, and that needs saying before anything else. What has changed is the understanding of the condition itself. Tendonitis, more accurately called tendinopathy, was long treated as an overuse injury with inflammation on top. Tendons are now known to be metabolically active tissue, and who develops persistent tendon pain tracks with metabolic and inflammatory factors far more than loading alone would predict.


The clue in who gets it


Tendinopathy is far more common in people with diabetes and in people carrying excess weight, including in tendons bearing no extra load at all.


That last detail is the informative one. If the condition were purely mechanical, extra weight would explain Achilles and knee tendon problems. It wouldn't explain raised rates in the shoulder and elbow, where body weight isn't a factor.


What excess weight and diabetes share is a raised background level of inflammatory signalling and altered handling of glucose and fats. Both affect how collagen, the structural protein tendons are built from, is made and maintained.


So the tendon is being affected by the chemistry it sits in, not only by the forces through it.


Where the gut enters, and how confident to be


This is inference from an established mechanism rather than direct evidence, and it's worth stating in those terms.


Gut bacteria are among the strongest influences on background inflammatory signalling. When the gut wall becomes more permeable, endotoxin from bacterial cell walls crosses into the bloodstream, and the immune system responds with low, continuous inflammation.


That process has been measured directly in nearby tissue. In 25 people with knee osteoarthritis, endotoxin levels in blood and in the fluid inside the joint tracked with several things at once: activated inflammatory cells in the joint lining, narrowing of the joint space, and higher pain scores.


Tendons attach to the same joints, sit in the same circulation and respond to the same inflammatory signals. Nobody has repeated that measurement in tendon tissue.


Gut bacteria also influence insulin sensitivity and how the body handles glucose, which is the other half of the metabolic picture in tendinopathy.

What actually treats tendon pain


Being honest about the gut evidence means being clear about what does work.


Progressive loading exercise is the treatment with by far the strongest evidence for tendinopathy. Tendons respond to controlled, gradually increasing load by remodelling and strengthening. Rest alone tends to make them worse.


That's slow work, often taking months, which is why people look for shortcuts. Injections and passive treatments generally perform worse than loading in trials.


Where the metabolic picture matters is in explaining why the same rehabilitation programme works better for some people than others, and why tendon problems cluster in people with metabolic conditions.


How much pain a given amount of tissue damage produces is itself variable. In widespread chronic pain, gut bacteria accounted for more of the difference between people than any other factor measured, including diet and activity.

What this means for you


Tendon pain that persists beyond a few weeks is worth assessing properly, and a progressive loading programme guided by a physiotherapist is the treatment with the strongest evidence behind it. In addition, the research above shows tendinopathy tracking with metabolic and inflammatory factors rather than loading alone, with gut-derived endotoxin measurably linked to inflammation and pain in nearby joint tissue.
