A genetic study links gut bacteria to reflux directly


Reflux and heartburn happen when stomach acid moves back up into the oesophagus, and researchers have now tested whether gut bacteria play a genuine causal role, not just a coincidental one. A large genetic study combined gut bacteria data from 18,340 and 7,738 people with reflux data from more than 602,000 people, and found real causal relationships between specific gut bacteria and both reflux and Barrett's oesophagus, a condition that can develop from long-term reflux.

The oesophagus has its own bacterial shift too


Separately from the gut itself, researchers have found the oesophagus carries its own distinct bacterial community. In a healthy oesophagus, Streptococcus makes up around 78.8% of the bacteria present. In people with reflux and Barrett's oesophagus, that share drops to around 30%. The gap is filled by a different group of bacteria, including Fusobacterium, Veillonella, Prevotella, and Campylobacter. One of these, Campylobacter, turns up in about half of people with reflux-related damage or Barrett's oesophagus, and its presence is linked to higher levels of a specific inflammatory marker, interleukin-18. This local shift is thought to be driven partly by the more acidic environment reflux creates, and partly by bacteria arriving from the mouth and stomach.

The standard treatment changes gut bacteria too


Proton pump inhibitors, or PPIs, are the most common medication for reflux. They work by reducing stomach acid. Reducing acid also changes which bacteria can survive in the gut. Studies have found PPI use lowers levels of two helpful bacteria groups, Bifidobacterium and Lactobacillus. At the same time, it raises levels of two others, Enterococcus and Streptococcus, which are more often linked to infection risk.


Researchers have tested whether adding a probiotic can help. In a registered clinical trial, patients took a probiotic alongside their usual PPI treatment. After they stopped the PPI, the probiotic group kept their symptom relief for longer than expected. Their gut bacteria showed a specific pattern: more Bifidobacterium, and more of a related bacterium called Lactiplantibacillus plantarum. Their gut also produced more of several helpful compounds, including short-chain fatty acids, the same compounds linked to gut healing elsewhere in this research.

Diet and lifestyle already matter here


One study following patients with Barrett's oesophagus found that low adherence to standard healthy lifestyle recommendations correlated with worse bacterial imbalance and higher measured cancer risk scores. A separate study, comparing tissue samples across five groups from healthy oesophagus through to early cell changes and confirmed cancer, found bacterial diversity fell in a stepwise pattern as the disease progressed further. This lines up with the wider gut research: what a person eats shapes gut bacteria, and gut bacteria appear to influence conditions well beyond the stomach and gut, including the oesophagus above it.

What this means for you


Frequent heartburn, difficulty swallowing, or reflux lasting more than a few weeks needs medical assessment, since Barrett's oesophagus needs monitoring once diagnosed.
