When Blood Sugar Drops Too Far After Eating


The research tying gut bacteria to this particular condition is thin, and it is worth saying so plainly. Reactive hypoglycaemia means blood sugar falling below the normal range within a few hours of a meal. It brings shakiness, sweating, a racing heart and trouble concentrating. The gut link runs through fermentation, the process where bacteria in the large intestine break down fibre the body cannot digest on its own. Fermentation produces compounds that influence the hormones controlling blood sugar. That mechanism is well established. What has barely been tested is whether feeding those bacteria changes the condition itself. Many people who assume they have it turn out, on proper testing, to have something else.


A diet study that changed the output, not the cast


One small study has looked at gut bacteria in people with this diagnosis directly. Researchers put 12 patients with reactive hypoglycaemia on one of two diets for three days. One was the Ma-Pi 2 diet, a macrobiotic eating pattern built around whole grains, vegetables and pulses. The other was a Mediterranean diet, used for comparison.


The bacterial line-up in each group did not converge on a shared pattern. Everyone kept their own individual mix. What did change was the output. Levels of short-chain fatty acids in stool rose significantly, but only in the group eating the Ma-Pi 2 diet. Short-chain fatty acids are the compounds bacteria make when they ferment fibre.


That distinction is the useful part. Three days was nowhere near enough to reshape which bacteria were present. It was enough to change what those bacteria were producing.

How fermentation reaches the hormones that steady blood sugar


That only counts for something if those compounds do a job. They do. Cells lining the lower gut release a hormone called glucagon-like peptide-1 (GLP-1), which prompts the pancreas to release insulin when blood sugar rises and slows how quickly the stomach empties. Laboratory work showed that short-chain fatty acids trigger those cells to release the hormone. The cells carry receptors that detect the compounds directly. Mice bred without one of those receptors released less of the hormone and handled glucose worse.


There is an honest complication here. In some forms of post-meal low blood sugar, particularly after stomach surgery, an oversized release of this same hormone appears to be part of the problem rather than the solution. So the aim is not simply more of it. The aim is slower, steadier delivery of sugar into the blood in the first place.

Evidence that fermentation smooths the next meal


Slower delivery is something fermentation has been shown to produce. In one study, 15 healthy adults ate different bread-based evening meals. Meals made with barley kernel bread improved how their bodies handled sugar at breakfast the next morning. Barley kernels carry fibre and resistant starch, a type of starch that escapes digestion in the small intestine and reaches the bacteria of the large intestine instead.


The researchers measured hydrogen in the breath as a marker of how much fermentation was going on. The more fermentation, the lower the blood sugar response at breakfast. Levels of that same gut hormone were higher when the blood sugar response was lower. The effect carried overnight, long after the evening meal itself had been digested.


This was done in healthy volunteers, not in people diagnosed with reactive hypoglycaemia. It shows the mechanism working. It does not show that the mechanism treats the condition.

What this means for you


The case here is modest, and it is better stated honestly than dressed up. Fermenting fibre produces compounds that slow and steady the delivery of sugar into the blood. One small study in people with this diagnosis showed a fibre-rich pattern raising those compounds within three days. Reactive hypoglycaemia should be confirmed by a doctor with proper testing, since several other conditions produce the same symptoms. Alongside that, meals built around whole grains, pulses, vegetables and nuts give the bacteria doing the fermenting far more to work with than refined carbohydrates do. Spreading those foods across the day matters more than perfecting any single meal, because the effect of one meal's fermentation turns up at the next one.
