A bacterium consistently missing in people with obesity


Akkermansia muciniphila, a bacterium that helps maintain the gut's protective mucus layer, is consistently found at lower levels in people with obesity, and animal research has shown it can help prevent diet-induced weight gain. This made it a genuinely promising, specific candidate for human testing, rather than a generic "good bacteria" claim.


A landmark experiment: obesity itself can be transplanted


In a striking series of experiments, researchers took gut bacteria from human twin pairs where one twin had obesity and the other didn't, and transplanted each twin's bacteria into separate groups of mice raised without any gut bacteria of their own. Mice that received bacteria from the twin with obesity gained significantly more body fat than mice that received bacteria from the lean twin, even though both groups ate the same low-fat diet, showing that the bacteria themselves, not just the person's habits, could directly cause weight gain.


When researchers then housed both groups of mice together, letting them naturally pick up each other's gut bacteria, the mice with "obese" bacteria stopped gaining excess weight, but only when they were also fed a diet high in fruits and vegetables and low in saturated fat. On a diet high in saturated fat and low in produce, this protective effect from picking up the leaner bacteria didn't happen. This is a genuinely important detail: the bacteria alone weren't enough to prevent obesity, the right food for those bacteria to work with was equally essential.

A real trial with a striking, specific result


A randomised controlled trial gave 90 adults with overweight or obesity an 8-week low-energy diet to lose at least 8% of their body weight, then followed them for 24 weeks on a normal, healthy diet, with half receiving daily pasteurised Akkermansia muciniphila and half a placebo. By the end of the maintenance period, the probiotic group had regained just 1.2kg on average, compared with 3.2kg in the placebo group, meaning the probiotic group kept off nearly three times as much of their initial weight loss.

As with any probiotic, this strain worked specifically in the context of an already-established healthy diet during the maintenance phase, not as a standalone weight-loss tool. Introducing helpful bacteria without a supportive diet to feed them and the rest of the gut gives them far less chance of producing this kind of lasting effect.


Works best for those who need it most


The trial found that people who started with lower levels of Akkermansia in their gut responded better to the treatment, a genuinely useful, specific insight suggesting this approach may be most valuable for those whose gut is most depleted of this bacterium to begin with, rather than a universal effect.


What this means for you


Obesity needs a comprehensive approach, and for many people this includes structured dietary changes, physical activity, and in some cases medication or surgery, guided by a doctor, none of which a probiotic replaces.
