When a Gut Bacterium Steals a Nerve-Protecting Vitamin
Neuropathy has many causes, and one of the most common and preventable involves a genuine competition between gut bacteria and the human body for a single vitamin. Vitamin B12 is essential for keeping myelin, the fatty protective coating around nerves, healthy and intact, and a deficiency is a well-established cause of neuropathy, the numbness, tingling, and pain that comes from damaged nerves. Metformin, the most widely prescribed medication for type 2 diabetes, is already known to lower B12 levels in a meaningful proportion of long-term users, with studies estimating anywhere from 5% to 40% of people on the medication affected, depending on dose and duration of use.
A specific bacterium caught competing for the vitamin
Researchers found that metformin doesn't just interfere with the body's own B12 absorption. It also changes the behaviour of a specific gut bacterium, Bacteroides ovatus, causing it to pull in and hoard more B12 for its own use. This directly increases competition between the bacterium and the human body for the same limited vitamin supply, worsening the deficiency beyond what metformin's other effects on absorption would cause alone. This is a genuinely specific, mechanistic discovery, not just a general statement that gut bacteria affect nutrient levels.

Confirmed in real patients, tied directly to nerve damage
That mechanism matters clinically. A cross-sectional study of 162 patients on long-term metformin found a genuine, measurable relationship between B12 deficiency and diabetic neuropathy, assessed using nerve conduction tests and a standard neuropathy screening tool. This connection is frequently missed, since B12 deficiency symptoms can closely resemble diabetic neuropathy from the diabetes itself, meaning some cases labelled as diabetic nerve damage may actually be a treatable vitamin deficiency instead.

Beyond metformin specifically, small intestinal bacterial overgrowth, or SIBO, where bacteria that normally live further down the gut overgrow in the small intestine, can independently cause B12 deficiency by consuming the vitamin before the body absorbs it, adding a second, distinct gut-related route to the same nerve-damaging endpoint.
A clinical trap that can hide the deficiency
There's an honest complication worth knowing about. The same overgrown bacteria that consume B12 in SIBO often produce folate, a related B vitamin, as a byproduct. This can push folate levels up at the same time B12 is being depleted, and doctors sometimes rely on folate levels as a rough proxy when assessing B vitamin status. A high folate result alongside low B12 is actually a recognised clue pointing toward SIBO specifically, rather than a reassuring sign that vitamin levels are generally fine.
