The Stress Route Into Persistent Neck Pain
Research on gut bacteria and neck pain specifically doesn't exist, and this page is built on established mechanisms rather than direct findings. What makes neck pain worth writing about anyway is that it behaves differently from most musculoskeletal pain. It correlates poorly with imaging, it travels with headache and jaw pain, and it tracks strongly with stress. That combination points at how pain signals are processed rather than at damaged tissue, and the gut sits inside that system.
Stress opening the gut barrier, measured directly
The first step of this chain has been produced deliberately in people.
Researchers asked 23 healthy volunteers to give a public speech, a standard laboratory method of inducing acute stress, and measured how permeable the small intestine was before and afterwards.
Permeability rose. The detail that matters is who it rose in. The effect appeared only in volunteers whose cortisol went up significantly. Those who stayed calm, measured by hormones rather than self-report, showed no change in their gut barrier.
Giving participants the hormone that triggers the stress response reproduced the same increase without any stressful task. Blocking a specific immune cell in the gut wall prevented it.
So the route from psychological stress to a leakier gut is established in humans, and it depends on the hormonal response rather than on feeling stressed.

Why a leakier gut affects how much something hurts
The second step is where inflammation meets pain perception.
When the barrier loosens, bacterial fragments cross into the bloodstream and the immune system responds with low, continuous inflammatory signalling. Those inflammatory molecules act directly on sensory nerves, lowering the threshold at which they fire.
That process is called sensitisation, and it's the best explanation for why the same amount of tissue strain produces very different amounts of pain in different people, and in the same person at different times.
The most direct evidence that gut bacteria participate comes from widespread chronic pain. Stool from women with fibromyalgia was transferred into mice, and those mice developed increased pain sensitivity. Mice receiving bacteria from healthy donors did not.
That doesn't concern neck pain. It does establish that a bacterial community can carry altered pain sensitivity into an animal that had none.

What this does and doesn't explain
Two clarifications belong here.
Neck pain has real mechanical causes. Poor positioning, sustained loading and injury all matter, and posture and movement remain the things most worth addressing.
What sensitisation explains is the part that mechanics don't. It explains why neck pain so often persists after the original strain has healed, why it flares during stressful periods without any change in activity, and why it travels with headache and jaw tension.
The chain from stress to gut barrier to inflammation to nerve sensitisation is established at each link separately. What hasn't been done is measuring it end to end in people with neck pain.
That makes this a reasonable inference rather than a finding, and it's worth reading as one.
What this means for you
Neck pain with arm weakness, numbness, or following a significant injury needs prompt medical assessment, and for ordinary neck pain staying active, addressing sustained positions and gentle movement have better evidence than rest or collars.
