What Gut Bacteria Have to Do With Migraine
Migraine is a common, disabling neurological condition, and it's substantially more common in women than men. Researchers have increasingly looked at the gut as a genuine piece of the migraine puzzle, not just a coincidence. Conditions like irritable bowel syndrome and colitis often occur alongside migraine, and gut symptoms are common among migraine patients generally. That overlap is exactly what pushed researchers to test whether changing gut bacteria could change migraine itself.
A real trial with a genuine reduction in attacks
A randomised, double-blind, placebo-controlled trial gave 79 people with migraine, both episodic and chronic, a multi-strain probiotic or placebo for 10 weeks. The probiotic group showed a significant reduction in migraine frequency and severity in both groups compared with placebo. In people with chronic migraine specifically, attack duration also decreased significantly. The placebo group showed no significant changes on any of these measures.

A distinct bacterial pattern, and an honest, mixed treatment picture
A systematic review comparing gut bacteria in migraine patients against people without migraine found significant, consistent differences in several major bacterial groups. The review's authors described this as a potential microbial signature that may be associated with migraine risk or how the condition progresses to a more chronic form. They were careful to note that the exact mechanism isn't yet clear, and that migraine medication itself may also affect gut bacteria, making it hard to fully separate cause from effect in these studies.
The same review found five randomised controlled trials showing that synbiotic or probiotic combinations significantly reduced migraine severity, frequency, duration, and painkiller use. Not every trial has agreed, and it's worth being upfront about that. A separate meta-analysis of two trials found a significant effect on migraine frequency specifically, but no significant effect on severity or attack duration.

As with any probiotic, this only works if the bacteria are properly fed. A strain taken to help manage migraine still needs fibre and plant compounds to establish itself well enough to keep influencing attack frequency over 10 weeks or longer.
A trial adding probiotics to standard medication
That inconsistency raises a practical question. Does a probiotic still help when added to standard treatment rather than tested alone? A double-blind randomised trial tested a multi-strain probiotic alongside sodium valproate, a standard migraine preventive medication, in 80 children aged 6 to 15 with migraine. Over four months, the combination produced a significant improvement in headache severity and frequency, and a significant reduction in daily painkiller use, compared with medication alone.
What this means for you
Migraine needs proper diagnosis, and standard treatments, including preventive medication and acute pain relief, remain the established first-line approach, particularly for frequent or severe attacks. Given the genuine, if mixed, trial evidence above, a specific multi-strain probiotic is a reasonable, low-risk addition to discuss with a doctor, especially if standard treatment isn't fully controlling attacks.
