A genuine biological mechanism: the "estrobolome"


Gut bacteria carry a collection of genes, sometimes called the "estrobolome," that reactivate oestrogen as it passes through the intestines and send it back into the bloodstream, effectively recycling a portion of the body's oestrogen supply. A meta-analysis with large effects, and an honest caveat about quality


A systematic review and meta-analysis of 7 trials comparing probiotics with placebo found large effects on total menopausal symptom scores, hot flashes specifically, psychological symptoms, and markers of vaginal health. The same review was honest that only about a third of the studies in its broader analysis were judged to have a low risk of bias, with a substantial share rated as having critical or high risk, meaning the size of these effects should be treated with real caution until confirmed in more rigorous trials.

As with any probiotic, the strains used in these trials only work if they're properly fed. A probiotic taken during menopause still needs fibre and plant compounds to establish itself well enough to support the estrobolome's recycling function, rather than just passing through.


A separate, more established use for bone health


Beyond symptom relief, some trials have found specific probiotic strains helped prevent bone mineral density loss at the spine and hip in postmenopausal women, though a broader review found this evidence to be weak, with very high variability between studies. Menopause symptoms significantly affecting quality of life are worth discussing with a doctor, since hormone replacement therapy remains the primary, most thoroughly evidenced treatment and a probiotic is not a substitute for it.
