More food alone often isn't enough


Childhood malnutrition has long been treated primarily as a calorie and nutrient problem, yet standard therapeutic foods often leave children stunted and underweight even after they've gained some weight back. Research has found a specific reason why. The gut bacteria of malnourished children are developmentally immature, resembling the microbiome of a much younger child, and standard treatments only partially and temporarily repair this, even when weight gain looks successful in the short term.

A landmark trial that beat a higher-calorie food


Researchers in Bangladesh designed a food specifically to repair this immature gut microbiota, built from locally available ingredients, rather than simply maximising calories. In a randomised controlled trial in children with moderate acute malnutrition, this microbiome-directed food produced significantly greater weight and height gains than a standard higher-calorie supplementary food, despite containing 15% fewer calories. The effect was closely tied to increases in specific gut bacteria, particularly certain strains of Prevotella copri, that correlated directly with how much the children grew.


Confirmed as cause, not just correlation


To test whether the microbiome itself was driving this effect, researchers transplanted gut bacteria from malnourished and healthy children into germ-free mice. Bacteria from malnourished children directly caused impaired weight gain, altered bone growth, and immune and metabolic abnormalities in the mice, confirming this wasn't just an incidental side effect of poor nutrition, but a genuine driver of it. A separate bacterium, Bifidobacterium longum subspecies infantis, is often depleted or missing entirely in children with severe malnutrition, and a randomised trial giving infants this specific strain improved weight gain and reduced markers of gut inflammation.

Malnutrition, whether from insufficient food, illness, or a medical condition affecting absorption, needs proper clinical assessment and, in children, close monitoring of growth over time, not just short-term weight gain.
