Age-related macular degeneration is the leading cause of irreversible vision loss in older adults in developed countries. The strongest evidence for any intervention comes from the Age-Related Eye Disease Study, a large randomised trial of 3,640 people that found a specific combination of vitamin C, vitamin E, beta-carotene, and zinc reduced the risk of progressing to advanced disease by 28%. What researchers have only more recently started to explore is why these nutrients help, and gut bacteria appear to be part of the answer.

A distinct gut bacteria pattern in people with the disease


Studies comparing people with macular degeneration to healthy controls have found real differences in gut bacteria, including reduced overall diversity and lower levels of Bacteroidota and Prevotellaceae specifically in people with the more severe, neovascular form of the disease. Researchers believe a disrupted gut barrier allows bacterial compounds to enter the bloodstream, triggering the same low-grade inflammation that's implicated in how the disease progresses, part of what's now called the gut-retina axis.

A trial testing whether nutrients work through the gut


A randomised trial tested whether the kind of micronutrient supplementation shown to help in the original AREDS trial actually shifts gut bacteria composition in people with macular degeneration, rather than only acting directly on the eye.
