A meta-analysis of 8 trials found probiotics and prebiotics raised haemoglobin levels in people with anaemia, alongside a genuine rise in serum iron.

A specific probiotic strain, tested directly
A meta-analysis of 8 randomised controlled trials, covering 632 people with anaemia, found that probiotic, prebiotic, or synbiotic supplementation significantly raised haemoglobin levels compared with placebo. Two of those trials, covering 120 people, also measured serum iron directly and found a significant increase in the supplemented group. One strain in particular, Lactobacillus plantarum 299v, has repeatedly been shown in controlled studies to increase how much iron the body absorbs from a meal, including in a randomised trial of 53 female athletes with low iron stores.

How gut bacteria help iron get absorbed
Only a small proportion of dietary iron is normally absorbed, which is why iron supplements often need to be taken at fairly high doses, and why they cause stomach upset in many people. Certain gut bacteria appear to help convert iron into a form the gut absorbs more easily, and lower the local pH in a way that also favours absorption. This gives a plausible mechanism for why a specific probiotic strain, taken alongside iron supplementation, has shown promise as a way to improve iron status without needing to push the iron dose itself even higher.
As with any probiotic, taking a strain on its own only introduces bacteria, it doesn't guarantee they establish themselves. Fibre and plant compounds are what keep a probiotic strain, and the rest of the gut, fed and functioning well enough to actually help with something like iron absorption.
Why the results aren't fully consistent
Not every trial has shown the same benefit. A study in paediatric patients found no added benefit from combining the same probiotic strain with standard iron supplements, and researchers have noted that trial quality varies considerably across this body of research, with small sample sizes and inconsistent measurement of iron markers like ferritin. This is a promising, plausible area of research rather than a settled one, and probiotics should be seen as a possible complement to iron treatment, not a replacement for it.
What this means for you
Iron deficiency needs a proper diagnosis through blood tests, since treating it based on symptoms alone risks missing both the deficiency and its underlying cause, which occasionally includes gut conditions such as coeliac disease or ongoing blood loss that need their own treatment. Persistent fatigue, pale skin, or shortness of breath are worth raising with a doctor. Where iron supplementation is prescribed, the evidence above is a reasonable basis for asking whether a specific probiotic strain alongside it might help, particularly if standard iron supplements cause troublesome stomach upset.
