A consistent effect across multiple meta-analyses


A meta-analysis of 17 randomised controlled trials found that probiotic consumption significantly reduced fasting glucose, fasting insulin, and HOMA-IR compared with placebo. A separate randomised trial testing a probiotic specifically in prediabetes, a 196-person, 12-week study, found meaningful improvements in gut bacteria composition alongside metabolic markers, with mild, comparable side effects between probiotic and placebo groups. The same study found a specific bacterium, Faecalibacterium prausnitzii, was directly linked to better insulin secretion, adding a second, distinct bacterial marker to the picture beyond Akkermansia alone.

A landmark experiment showing how a leaky gut causes it


A separate, direct mechanism helps explain why gut health and insulin resistance are so closely linked. A landmark study found that a bacterial fragment called lipopolysaccharide, which normally stays contained within gut bacteria, can leak into the bloodstream when the gut barrier is compromised, a state researchers call metabolic endotoxemia. When researchers infused this fragment into mice on an otherwise normal diet, it directly caused both obesity and insulin resistance to develop, a genuinely direct demonstration of cause and effect, not just an observed association. This fragment triggers inflammation by activating a specific immune receptor, and that inflammation directly interferes with the cell signalling insulin depends on to work properly.


This mechanism has since been confirmed directly in people, not just mice. Researchers gave healthy human volunteers a low, controlled dose of this same bacterial fragment and measured a 35% drop in insulin sensitivity within hours, alongside increased inflammatory markers in fat tissue, while pancreatic insulin-producing cells kept working normally. This is a rare, genuinely direct experiment in humans showing that this specific gut-derived compound alone is enough to cause measurable insulin resistance, not merely something that happens to occur alongside it.

As with any probiotic, this only works if the bacteria are properly fed.
