Hypothyroidism, an underactive thyroid, is most commonly caused by Hashimoto's thyroiditis, an autoimmune condition, and most patients are treated effectively with levothyroxine replacement therapy. A modest, real, but limited effect from probiotics


A meta-analysis of 8 randomised controlled trials examining probiotic or prebiotic supplementation and thyroid function found a statistically significant change in free T4, one of the main thyroid hormones, though TSH, the pituitary hormone that regulates thyroid output, didn't change significantly. This is an honest, partial result: a genuine effect on one relevant hormone marker, but not a demonstrated fix for the underlying condition.


A specific trial with a genuine quality-of-life benefit


A 2025 double-blind trial testing the probiotic Lactiplantibacillus plantarum 299v alongside nutritional education in women with Hashimoto's thyroiditis found the probiotic enhanced the quality-of-life benefits of the nutritional intervention, a clinically meaningful outcome for a condition that significantly affects daily wellbeing even when hormone levels are well managed. As with any probiotic, this only works if the bacteria are properly fed. A strain given alongside thyroid treatment still needs fibre and plant compounds to establish itself well enough to genuinely support hormone recycling, rather than just passing through.

Hypothyroidism needs proper diagnosis and levothyroxine treatment where indicated, with regular blood tests to confirm dosing is correct, and no probiotic replaces this.
