Bile acids are the mechanistic link
Gut bacteria influence cholesterol levels through a genuinely well-understood mechanism: certain bacteria break down bile acids in the gut, and since bile acids are made from cholesterol in the liver, this breakdown pushes the liver to convert more cholesterol into new bile acids to keep up, lowering the cholesterol available to circulate in the blood. This is a specific, mechanistic pathway, not just a loose correlation.
Consistent reductions across many trials
A meta-analysis of 41 randomised controlled trials examining gut microbiome-targeted interventions, including probiotics and prebiotics, found significant reductions in total cholesterol and LDL cholesterol, the type most strongly linked to cardiovascular risk. A separate, earlier meta-analysis focusing specifically on Lactobacillus strains found that particular species, including L. reuteri and L. plantarum, reduced total and LDL cholesterol significantly, while having little effect on HDL, the protective type.

As with any probiotic, the strains behind these reductions only work if they're properly fed. Introducing helpful bacteria without also supplying fibre and plant compounds gives them far less chance of maintaining the bile acid breakdown that drives this cholesterol-lowering effect.
An honest caveat on HDL and triglycerides
Across this body of research, probiotics reliably move total cholesterol and LDL, but effects on HDL and triglycerides are far less consistent, with high variability between studies. This is a useful, honest distinction: gut bacteria appear to have a genuine, specific effect on harmful cholesterol clearance, without necessarily improving every lipid marker equally.
What this means for you
High cholesterol needs proper diagnosis through blood testing, and for many people, statins or other prescribed medication remain the most effective, well-evidenced treatment, particularly for those at higher cardiovascular risk.
