A promising small pilot trial found a probiotic improved heart function, but a larger, better-designed follow-up trial found no such effect, an honest example of science self-correcting.

A genuine, well-mapped biological connection

A promising pilot, and an honest larger trial
A small pilot trial of 20 patients found that the probiotic yeast Saccharomyces boulardii improved heart pumping function, measured as ejection fraction, more than placebo, a genuinely exciting early result. A much larger, well-designed follow-up trial, called GutHeart, then tested the same probiotic, alongside a gut-targeted antibiotic, in 151 patients with heart failure. This larger trial found neither treatment significantly improved heart function, TMAO levels, or even gut bacteria diversity compared with standard care alone.

An honest, useful null result
The researchers behind the larger trial suggested one reason for the different outcome: their patients were already well treated for heart failure with modern medication, and had relatively mild gut bacteria disruption to begin with, meaning there may have been less room for a gut-focused treatment to show a benefit. This is a genuinely useful, honest example of how a promising small trial doesn't always hold up, and it's important to report that plainly rather than only sharing the exciting early result.
What this means for you
Heart failure needs ongoing, careful management by a cardiologist, with medication adjusted based on symptoms and heart function tests, and this remains true regardless of gut-focused research. Based on the best current evidence, a specific probiotic shouldn't be expected to meaningfully improve heart function in people already on optimal heart failure treatment.
