What Gut Bacteria Explain About Heart Risk


The research here is solid, built on large population studies, though no trial has yet changed anyone's bacteria and measured what happens to their heart. Heart disease develops out of a set of measurable risk factors, including blood fats, blood pressure and body weight. Gut bacteria influence some of those factors and not others. That selectivity is the interesting part, because it points at which mechanisms are really involved rather than suggesting the gut affects everything at once. Most accounts treat gut health and heart health as loosely related. The numbers are more specific than that.


What the bacteria explain, and what they do not


Researchers studied 893 people from a large Dutch population cohort. They sequenced gut bacteria and measured body weight and blood fats, then worked out how much of the variation between people the bacteria could account for.


Thirty-four bacterial groups were associated with body mass index or blood fats. Once age, sex and inherited genetic risk were accounted for, gut bacteria explained about 4.5% of the differences in body mass index between people. They explained about 6% of the differences in triglycerides, a type of fat carried in the blood. They explained about 4% of the differences in HDL cholesterol, the form usually described as protective.


Then comes the result that makes this credible. Gut bacteria explained almost nothing about LDL cholesterol or total cholesterol.


That matters. LDL cholesterol is the blood fat most strongly tied to heart disease and most responsive to statin medication. A finding showing gut bacteria affecting every measure equally would look like noise. A finding that separates triglycerides from LDL looks like a real, specific biological effect.

Following people forwards rather than photographing them once


Explaining variation in a risk factor is not the same as predicting an event. For that, people have to be followed over time.


Researchers recruited 211 patients who already had coronary artery disease, high blood pressure, raised blood fats or diabetes. Their gut bacteria were sequenced and sorted, without reference to their health, into two groups based on bacterial composition alone.


The two groups turned out to differ. One had greater bacterial variety. It also contained younger people with lower body weight and less coronary artery disease.


Over a median of 7.4 years, that group had roughly half the rate of serious cardiac events, hospital admission for heart failure and death compared with the other group.


The caution attached to this is real. Those groups differed in age, weight and existing disease, so the bacteria are not the only thing separating them. It is a signal worth following rather than proof of cause.

What would settle it, and why nobody has done it


One gap runs through everything above, and it is worth naming plainly.


Every study described here observed people as they were. None deliberately changed anyone's gut bacteria and then counted heart attacks years later. That trial would need thousands of participants followed for a decade, and it has not been run.


So the honest position is this. The associations are consistent and they appear in large groups. They survive adjustment for genetics and age. They also behave selectively, in a way that suggests real biology rather than statistical noise. What they cannot yet tell us is how much of a person's heart risk would actually shift if their gut bacteria changed.


What this means for you


Putting it together, gut bacteria account for a modest but real share of the differences between people in body weight, triglycerides and HDL cholesterol, and populations with more varied gut bacteria have gone on to have fewer cardiac events. That share is meaningful without being the main story. Blood pressure checks, cholesterol testing and prescribed medication remain the parts of heart care with the strongest evidence behind them. Alongside those, the case for plant variety here is that the measures gut bacteria did move, triglycerides and HDL, are the ones fibre-fermenting bacteria influence through the compounds they produce.
