A striking finding in germ-free mice
Glaucoma, the second leading cause of blindness worldwide, involves progressive damage to the optic nerve, most often linked to elevated pressure inside the eye. A genuinely striking mouse experiment found that germ-free mice, raised with no gut bacteria at all, were resistant to the glaucomatous nerve damage that developed in normal mice under the same conditions. This is a rare, direct piece of evidence that gut bacteria aren't just a bystander in this disease, but actively contribute to the nerve damage itself.

A specific bacterial signature in human patients
Large population studies have found real differences in the gut bacteria of people with glaucoma. Using data from the Rotterdam Study, a major long-running population cohort, researchers found that people with glaucoma had a lower abundance of butyrate-producing gut bacteria, and this was linked to both eye pressure and the severity of optic nerve damage. Other studies have found glaucoma patients tend to have higher levels of specific bacteria, including Prevotellaceae and E. coli, though the exact bacterial signature varies somewhat between studies.

A plausible route from gut to eye
Researchers believe gut bacteria and their byproducts, including inflammatory compounds and reactive oxygen species, can travel from the gut to the eye via the bloodstream, reaching the optic nerve and the eye's drainage structures. A separate study found increased intestinal inflammation and a leakier gut lining in people with glaucoma compared with healthy controls, adding more direct human evidence to the mouse findings above, though it doesn't yet prove which comes first.
What this means for you
Glaucoma often has no symptoms until vision loss has already begun, so regular eye pressure checks, particularly for anyone over 40 or with a family history, remain the most important step for catching it early. Prescribed eye drops and other treatments to lower eye pressure are the proven way to slow the disease and should never be stopped or adjusted without a doctor's guidance.
