An overgrowth of gut bacteria can cause folate levels to look artificially high, a genuine diagnostic clue doctors use to help spot the problem.

Bacteria that make folate, not just use it
Folate, or vitamin B9, is essential for DNA synthesis and cell division, and unlike most nutrients this site covers, gut bacteria don't just help absorb it, some of them make it directly. A systematic review of the human gut microbiome found that bacterial folate synthesis genes are widespread, with common genera including Bifidobacterium, Lactobacillus, and Streptococcus among the producers. Around 13% of gut bacterial genomes carry the complete set of genes needed to make folate entirely from scratch, and many more can contribute if given a key building block.

A useful diagnostic paradox in SIBO
This bacterial production creates a genuinely useful clinical clue. In small intestinal bacterial overgrowth, or SIBO, where bacteria multiply excessively in the wrong part of the gut, the sheer number of bacteria can push measured folate levels higher rather than lower. Doctors sometimes use a pattern of high folate alongside low vitamin B12, another nutrient affected by gut bacteria but in the opposite direction, as a diagnostic clue pointing toward SIBO specifically, rather than a general nutrient deficiency.
Why this doesn't mean bacteria reliably prevent deficiency
Despite bacteria's genuine capacity to produce folate, an observational study of 200 adults found that how much folate a person's gut bacteria could produce in the lab didn't predict their actual blood folate levels. This is a useful, honest finding. A bacterium's theoretical ability to make a nutrient doesn't automatically translate into a measurable difference in how much reaches general circulation, since location in the gut, competition between species, and how much of that folate the body can actually absorb all matter too.

What this means for you
Folate deficiency causes fatigue, mouth ulcers, and, in pregnancy, is linked to a higher risk of neural tube defects in the developing baby, which is why folic acid supplementation is specifically recommended before and during early pregnancy regardless of diet. Persistent fatigue or unexplained mouth soreness are worth a blood test rather than assuming a cause.
