A disease driven by oestrogen, and a gut connection to match


Endometriosis, where tissue similar to the womb lining grows outside the uterus, affects 5 to 15% of women of reproductive age and is strongly driven by oestrogen. Certain gut bacteria carry genes that help regulate how oestrogen is broken down and recirculated in the body, a collection of genes researchers call the "estrobolome." Since endometriosis growth depends so heavily on oestrogen exposure, an altered estrobolome is a genuinely plausible way gut bacteria could influence the disease, not just an incidental association.

A consistent bacterial pattern, though evidence remains mixed


Reviews of the available research describe a fairly consistent pattern in women with endometriosis: reduced levels of beneficial bacteria, including Lactobacillus and Bifidobacterium, alongside increased levels of some potentially pro-inflammatory bacteria. That said, a systematic review covering the full body of research on this topic was honest that no single, consistent microbial or estrobolome "signature" has yet been firmly established across studies, and one small case-control study found no significant difference in overall gut bacterial diversity between women with and without the condition, despite finding differences in specific estrogen metabolites.

A second route through inflammation


Beyond oestrogen, a disrupted gut barrier can allow bacterial compounds called lipopolysaccharides into the bloodstream, activating an inflammatory signalling pathway that's been separately linked to worsening endometriosis symptoms. This gives a second, independent route by which gut bacteria could plausibly affect the disease, alongside the oestrogen-metabolism angle.


What this means for you


Endometriosis needs proper diagnosis, usually through imaging or laparoscopy, and treatment from a gynaecologist familiar with the condition, since delayed diagnosis is common and pain shouldn't be dismissed as ordinary period pain. The gut-oestrogen connection described here is a genuinely interesting, biologically plausible research direction, but the evidence isn't yet consistent or strong enough to support a specific gut-focused treatment recommendation.
