A distinct microbial fingerprint in diabetic retinopathy
Diabetic retinopathy, damage to the blood vessels of the retina caused by diabetes, is a leading cause of vision loss in working-age adults. A systematic review and meta-analysis pooling 18 studies found that people with diabetic retinopathy have a gut bacterial community with a distinctly different overall structure compared with people who have diabetes but no eye damage, and compared with healthy people, even though the sheer number and evenness of species present didn't consistently differ between groups.

Specific bacteria identified through deeper sequencing
A separate meta-analysis of 7 case-control studies, covering 195 people with diabetic retinopathy and 211 with diabetes alone, confirmed a genuine association between gut bacteria composition and both the presence and progression of retinal damage. Using more detailed shotgun sequencing, one study identified specific species, including Eubacterium hallii and Alistipes finegoldii, enriched in people with retinopathy, alongside altered metabolic pathways in their gut bacteria.

A plausible route from gut to retina
Researchers believe a disrupted gut barrier allows bacterial toxins into the bloodstream, triggering the low-grade inflammation and blood vessel changes that damage the retina, sometimes described as a gut-eye axis. This fits with what's already known about diabetes and gut bacteria more broadly, since gut bacterial changes are a well-established feature of type 2 diabetes itself, even before retinal damage develops. Researchers have proposed that restoring a healthier gut bacterial balance, through diet, probiotics, or other approaches, could become a genuine complementary strategy for reducing retinopathy risk, though this remains a research direction rather than an established treatment at this stage.

Diabetic retinopathy often causes no symptoms until damage is advanced, so regular diabetic eye screening, as recommended by a diabetes or eye care team, is essential regardless of how well vision seems to be at any given time. Good blood sugar control remains the single most effective way to reduce the risk of retinopathy developing or progressing, and this research doesn't change that.
