How a Leaky Gut Shapes Dengue's Severity


The evidence here is moderate, built on a mix of human observation and a confirming animal experiment.


Dengue is a mosquito-borne viral infection common across tropical regions, and most cases are mild, but a smaller number progress to a severe, sometimes life-threatening form.


Gut bacteria appear to shift the odds of which path an infection takes.


The gut becomes measurably leakier as dengue gets worse


A study measuring gut permeability directly in dengue patients found signs of a leaky gut, meaning the intestinal lining lets more through than it should, present from as early as the fourth day of fever.


The same study tracked patients through the febrile phase, around 4 days into illness, and into the critical phase, around 6 to 7 days in, when severe dengue typically develops if it's going to.


Patients in the more severe, critical phase of the illness had a different mix of bacteria detectable in their blood compared with healthy people, with more Bacteroidetes and E. coli, and less Bifidobacterium, a genus generally considered beneficial.


This leakage lined up with markers of gut cell damage and a stronger inflammatory response, the kind linked to the plasma leakage and organ stress seen in severe dengue.

Depleting gut bacteria in mice made a related virus infection worse


Dengue belongs to a family of viruses called flaviviruses, which also includes Zika and West Nile virus, and a mouse study tested what happens when gut bacteria are removed before infection with viruses in this family.


Mice given oral antibiotics beforehand, wiping out much of their gut bacteria, went on to develop significantly worse disease once infected, compared with mice whose gut bacteria were left intact.


This is a stronger form of evidence than the human observational data alone. Deliberately removing gut bacteria and watching the same virus cause worse disease shows the bacteria were doing something protective, not just present alongside a milder course by coincidence.

What this means for you


Dengue needs medical monitoring, especially for warning signs like severe abdominal pain, persistent vomiting, or bleeding, since severe dengue can progress quickly and needs hospital care.
