Why This Common Bacterial Shift Raises Infection Risk
There's strong evidence here, built on a large meta-analysis rather than a single study.
Bacterial vaginosis (BV) happens when the vaginal community shifts away from Lactobacillus dominance toward a more diverse mix of other bacteria, and it's the most common vaginal condition in women of reproductive age.
Beyond its own symptoms, BV measurably raises the risk of catching several other infections at the same time.
A large meta-analysis found a real increase in HIV risk
A meta-analysis pooling 23 studies and more than 30,000 women found BV increased the risk of acquiring HIV by approximately 60%.
The same body of research links BV to a higher risk of several other sexually transmitted infections, including gonorrhoea, chlamydia, trichomoniasis, genital herpes, and HPV.
BV is not itself classed as a sexually transmitted infection, which makes this risk-amplifying role easy to overlook. It changes the local environment in a way that makes other infections more likely to take hold, rather than being one itself.

There's a specific, testable mechanism behind the HIV risk
Researchers have identified a plausible reason for this, rather than leaving it as an unexplained statistical link. Genital fluid samples from women with BV were found to activate a receptor called TLR2, part of the immune system's early-warning system, on immune cells called dendritic cells.
This activation increased the cells' production of inflammatory signals and their ability to support HIV infection and onward transmission when tested directly against the virus in the laboratory.
This gives a genuine, mechanistic route from a bacterial shift in the vagina to a measurably higher chance of HIV taking hold, not just bacteria and virus happening to occur in the same place.

The same bacterial shift raises risks during pregnancy too
A meta-analysis pooling 18 studies and over 20,000 pregnant women found BV more than doubled the risk of preterm delivery, birth before 37 weeks of pregnancy.
The risk was highest when BV was present early on, before 16 weeks of pregnancy, when it was linked to a much steeper rise in preterm delivery risk than when detected later on.
The same analysis found a similarly raised risk of spontaneous miscarriage. This fits the same overall pattern seen with HIV and other infections. A bacterial shift in one location changes the odds of several different, seemingly unrelated outcomes.
