A direct mechanism, not just an association

Higher TMAO tracks with more severe disease
Separate research found that TMAO levels were significantly higher in patients with persistent atrial fibrillation, the more advanced, harder-to-treat form, compared with those whose atrial fibrillation came and went, and each unit increase in TMAO was linked to meaningfully higher odds of having the more persistent form. A systematic review of studies in this area confirmed a consistent link between gut bacteria imbalance and atrial fibrillation progression, though it noted that not every study measuring TMAO specifically has found the same result, an honest inconsistency worth acknowledging.
