The Gut's Part in Alcohol Craving


A randomised trial has tested this connection directly, which is unusual for a condition normally studied as a disorder of the brain alone. Heavy drinking damages the gut wall and reshapes which bacteria live there. Those changes appear to feed back into how strongly a person craves alcohol, and into the low mood and anxiety that so often accompany trying to stop. That is a different framing from the usual one. It suggests some of what makes stopping hard is being generated in the gut rather than only in the mind.


Why some people struggle more after detox


Not everyone finds withdrawal equally difficult, and researchers wanted to know what separated them.


They studied people who were alcohol dependent and going through a short detoxification programme, measuring how permeable their gut wall was. The gut wall normally forms a tight barrier. Heavy drinking loosens it.


Participants split into two groups. Some had a gut wall that had stayed relatively intact. Others had a markedly leakier one, and those people also had a different bacterial community, with fewer of the bacteria that ferment fibre.


At the end of the programme the difference showed. Those with the leakier gut wall still had high scores for depression, anxiety and alcohol craving. Those whose barrier had held up did not.


Both groups had stopped drinking for the same length of time. What differed was the state of the gut, and the feelings that make returning to alcohol more likely.

A trial that changed craving


An association like that invites an obvious experiment. Change the bacteria and see whether craving changes.


Researchers ran a placebo-controlled randomised trial in 20 men who had both alcohol use disorder and cirrhosis, meaning advanced liver scarring. Half received a faecal microbiota transplant, a procedure transferring stool from a healthy donor into someone else's gut to introduce their bacteria. Half received a dummy treatment.


The donor was chosen for being rich in two bacterial families, Lachnospiraceae and Ruminococcaceae, that were depleted in the participants. Both families are major fibre fermenters.


Nine of the ten who received the transplant reported reduced craving and drank less. Three of the ten given the dummy treatment did. Short-chain fatty acids, the compounds bacteria make from fibre, rose in the transplant group and not the placebo group. Over the following six months, alcohol-related problems were fewer in the transplant group.


This was a phase 1 safety trial with 20 participants. It shows the effect is worth pursuing, not that the treatment is ready.

Passing the effect to animals that never drank


Human trials cannot easily separate a bacterial effect from expectation. So the same team took the question to mice.


They collected stool from the trial participants, before and after their transplants, and used it to colonise mice raised with no gut bacteria of their own.


Mice given the post-transplant stool drank less alcohol and showed less preference for it than mice given the pre-transplant stool. The specific bacteria that had increased in the human participants were the ones associated with lower drinking in the mice.


The controlling detail is what happened with the sterile version. When researchers used filtered liquid containing the bacterial products but no live bacteria, the effect disappeared. Living, established bacteria were required.


That last point carries a practical implication. Bacteria introduced into a gut have to survive there, and the families involved here live on fermented fibre. Without food reaching them, an introduced population thins out and stops producing anything.

What this means for you


The chain here is more complete than for most conditions. A damaged gut barrier goes with worse craving and mood after stopping, restoring the missing bacteria reduced craving in a randomised trial, and the effect transferred to animals only when live bacteria were used. Reducing alcohol intake substantially or stopping altogether should be planned with a doctor, because withdrawal from heavy dependent drinking can be dangerous and sometimes needs medical supervision. Alongside proper treatment and support, the food-first reasoning points somewhere specific rather than general, since the bacteria depleted in these studies and enriched in the successful donor were fibre fermenters.
